December 31, 2008: deadline for 2008 fee payment, along with a penalty of 20% of the determined fee, for all provinces except Ontario.
December 31, 2008: deadline for submission of interim payment applications in Ontario.
March 31, 2009: deadline for submission of interim payment applications, in all provinces except Ontario and Alberta.
September 30, 2009: deadline for submission of the 2008 Harmonized form without penalty, in all provinces.
December 31, 2009: deadline for submission of the 2008 Harmonized form, with a penalty of $500 per month reduction of payment, for all provinces except Ontario.
